Charlotte White Center, CWC, Acquired Brain Injury      Our residential services for adults with acquired brain injuries are licensed by the State of Maine, and are one of the only fully accredited agencies by the Commission on Accreditation of Rehabilitation Facilities (CARF).  We currently provide rehabilitation services in several homes in Dexter, Dover-Foxcroft, Glenburn and Levant.

 

Eligibility

  • Referred to CWC by Maine DHHS/OACPDS, brain injury services

  • 18 years of age or older

  • Diagnosis of acquired brain injury

  • MaineCare or be eligible for it

 

Services Provided

  • Statewide Care Coordination

  • Rehabilitation treatment

  • Assistance or supervision with: daily living activities, which includes bathing, dressing, eating, toileting, ambulation, personal hygiene activities, grooming, performance of incidental household tasks essential to the activities of daily living, maintenance of his/her health and safety, administration of physician ordered medication

  • Personal supervision to ensure his/her health and safety

  • Arranging transportation and making phone calls for appointments as recommended by medical providers or as indicated in his/her plan of care
